The package consists of a blue medical report of the size of 17 x 22.50 cm. approximately, with the logo of the clinic with initials C N T M (Cronaca Nera, Tissa Mawartyassari) closed by a nurse's sock with the Red Cross logo and a brooch with the nursing symbol. Nursing symbols are of four different types with abbreviations RN, MA, PA, NP.
Inside there are two questionnaires of the clinic C N T M to ascertain erectile dysfunction, and three photographs of "special" nurses to conduct excitement exercises.
The CDR and the cover are contained in a white envelope.
